Case Integrity Division: Investigates compliance and fraud issues in all public assistance programs, medical assistance programs and child day care programs. NY Connects Long Term Care Office - Boyle Center, 149 Genesee St., Auburn, NY 13021 Services include information and referral, intake and screening, home assessment/evaluations, care plan development, case management and community PRI assessment (screening for nursing home placement), and access to Medicaid home care programs and Offi ce for Aging programs such as caregiver support, respite and EISEP (home care). Mission: Assisting the public in filing applications for social security benefits.
