Mission: The facility's mission is to maximize our resident's rehab potential and possibly transfer to least level of care through skilled nursing care, short-term rehab, social services and psychological services.

Services: The 92-bed skilled nursing home facility offers physical therapy, occupational therapy, speech therapy, audiology, dental, activities, podiatry and Hospice care.

Hours: Office - Monday through Friday, 8am - 5pm; Visiting - Daily, 9am - 10pm
